REQUEST FOR QUALIFICATIONS FORSMALL PROJECT CONTRACTORS PROGRAMMANDATORY PRE-QUALIFICATION MEETINGJANUARY 19, 2012 ATTENDANCE SIGN-IN SHEET • No RFQ submission will be accepted from a company not represented at this Mandatory Pre-qualification Conference. • The sign-in sheet will be the sole proof of representation, so please be sure to provide all info requested on the sheet! • Copies of sign in sheet will be posted online.

REQUEST FOR QUALIFICATIONS FORSMALL PROJECT CONTRACTORS PROGRAMMANDATORY PRE-QUALIFICATION MEETINGJANUARY 19, 2012 RFQ PURPOSE AND TERM This program pre-qualifies contractors to submit bids for small projects over the next 2 years in the following categories: • Electrical • Painting • Plumbing • Roofing • Fire Sprinkling • Telecommunications • Air Conditioning • General Contractor Contractors who qualify will perform work with not less than 75% of their own forces, exception, General Contractor.

REQUEST FOR QUALIFICATIONS FORSMALL PROJECT CONTRACTORS PROGRAMMANDATORY PRE-QUALIFICATION MEETINGJANUARY 19, 2012 MINIMUM QUALIFICATIONS (except painting) • Must be in continuous business for the past three years • Must have completed a minimum of at least $1,000,000 worth of work in the last three years in the respective category • Must have a minimum of five full time employees

REQUEST FOR QUALIFICATIONS FORSMALL PROJECT CONTRACTORS PROGRAMMANDATORY PRE-QUALIFICATION MEETINGJANUARY 19, 2012 MINIMUM QUALIFICATIONS FOR PAINTING • Must be in continuous business for the past three years • Must have completed a minimum of at least $500,000 worth of work in the last three years in the respective category • Must have a minimum of three full time employees

REQUEST FOR QUALIFICATIONS FORSMALL PROJECT CONTRACTORS PROGRAMMANDATORY PRE-QUALIFICATION MEETINGJANUARY 19, 2012 AWARDING OF WORK When needed work has been identified by the Authority, a scope of work will be developed and provided to the Contractors, having been previously qualified in the work category, to submit bids. A sample contract is included in the Solicitation.

REQUEST FOR QUALIFICATIONS FORSMALL PROJECT CONTRACTORS PROGRAMMANDATORY PRE-QUALIFICATION MEETINGJANUARY 19, 2012 INSURANCE REQUIREMENTS • Please refer to section 18.0 for complete insurance requirements. • In summary, minimum insurance requirements are: • $100K / $500K / $100K Workers’ comp • $1 million General Liability • $1 million Auto Liability

However, W/MBE participation is encouraged where possible for materials or supplies.
