  2. What is the PDC Pilot? • Collaborative • Systematic approach to quality improvement • Test and measure innovations • Share experiences to accelerate learning • Widespread implementation of best practices • Team Resources

  3. What makes a collaborative project? • Good ideas (change package) • Focus on results (monthly reports) • Peer support (everybody teaches, everybody learns) • Action-oriented

  4. What makes a team successful? • Leadership • Relationships • Alignment with organizational mission • Tests of change • Measurement

  5. What to expect from today’s session • Model for Improvement • Overview of Website • Review of change packages • Conference Call Schedule • Next Steps

  6. Next Steps • The time between calls is called an Action Period • Conduct tests of change • Implement and spread improvements • Measure and report results • Share with others • Attend conference calls

  7. Monthly Reporting • Monthly turnover reports are due by the 2nd Friday of the month. • Opportunity to share the successes and challenges you encounter.

  8. Remember confidentiality • Cannot share information about or from Pilot peers without their permission; NO EXCEPTIONS • Participants are expected to share and borrow from each other freely

  9. What to expect from future calls • Refine understanding of the elements and focus on team sharing and interaction (teams are experts for each other) • Brief Presentation on each domain • Sharing of lessons learned

  10. PDC Schedule • Next call-Monday, January 23 at 2 pm. Dial-in number: 1-800-262-1292 Report on Change Package results and which domain each facility will begin with. • Webex calls-2nd Monday of each month at 2 pm CST. (Posted on website) 2-13-06 • E-mail reminder-sent week prior to the call • Turnover reports due monthly by the 2nd Friday of the month. All team members are strongly encouraged to attend calls.
