Due tothe gaining popularity in the last decade or so,Chemical Peeling is a service seen to be offered bysalons, so-called cosmetologists and even ourneighbourhood hair parlours. Ideally, chemicalpeeling should be done under the supervision of aqualified dermatologist only. What is Chemical Peeling? It is a procedure whereby a chemical in low concentration is used over the skin to cause controlled chemical burn. How does it work? In superficial peels the mechanism of action is by coagulation of protein present in the superficial layer of epidermis. This action causes the actual peeling of the superficial layersof epidermis and subsequent regeneration that gives the desired effect. The medium peels promote epidermal regeneration and collagen remodelling. Skin lightening due to peels occurs due to inhibition of enzyme involved in melanin (pigment forming cell) synthesis. Certain peels also have anti-inflammatory action, comedolytic andlocal anaesthetic action. What are the common agents in Chemical Peeling? Common agents used for chemical peels are glycolic acid, salicylic acid, tricholoroaceticacid, combination peels. For what skin conditions can chemical peeling be used? The common indications of chemical peeling: - Superficial acne scars - Presence of inflammatory acne lesions (Grade II to Grade III)

Patients with realistic expectations b. Patients who are compliant with respect to follow-ups and adherence to instructions c. Chemical peels can be avoided in cases of facial hyperpigmentation and acne in pregnant women as these conditions are related to hormonal changes during pregnancyand are likely to subside post delivery. d. Chemical peeling should be avoided in patients with active infection or wounds at the siteof peeling, patients with keloidal tendencies and patients with unrealistic expectations. What are the side effects of chemical peeling? a. Irritation and burning at the site of peeling. b. Erythema or redness that may persist for 2-3 days post peel. c. Reactivation of herpes infection or secondary bacterial infection at the site of peel. d. Dispigmentation (hyper or hypo pigmentation following peel).
