  • Divisions – Bayside GP Network / Monash – 9 practices

  • Major partners to be engaged – Southern Health, RDNS, Local Govt

  • Other agencies and services which express interest in participating

    • BAEW – promoting physical activity and healthy eating for children in KB

    • Active for Life – physical activity, falls prevention and older persons

    • Talking Realities – peer education program for single young mothers
