Submittal Instructions • Proposals must be received by Monday, November 18, 2013 by 12:00PM • Submit one(1) proposal with original signature six (6) copies and one electronic copy in PDF Form on a flash drive or CD to: COWIC Administrative Offices 1111 East Broad St., Suite 201 Columbus, Ohio 43205 Attention: Winford Dearing, Procurement Manager • Late proposals will not be accepted under any circumstances

  4. Proposal Format and Content The proposal should consist of the following information in the prescribed order: 1. Proposal Check sheet-Indicate by checking the boxes that the form is attached 2. Proposal Cover Sheet-Attachment 3 3. Executive Summary-(Part of Attachment 3)Not to exceed one page. In the executive summary please detail the history and the mission of the organization. Description of how the proposed program is in alignment with the organizations primary mission and goals. 4. Service Provider Activity –Attachment 4 5. COWIC’s Disclaimer and General Provisions 7. Program Budget Justification, Budget Narrative and include Cost Allocation Plan (Budget forms are located on COWIC’s website).

  5. Proposal Format and Content The proposal should consist of the following information in the prescribed order(Continued) 8. Certification regarding Debarment Suspension- On Website 9. Certification regarding Lobbying -On Website 10. Provision Regarding Drug Free Workplace-On Website 11. Non-Collusion Affidavit -On Website 12. Status letter from Secretary of State 13. Most Recent Audit for the past two years( including supplemental schedules and management letter) 14. Quality Assurance plan 15. Tax Identification Number and Certification (IRS form w-9) Note: A full line of business insurance will be required of all successful bidders.

Proposal Narrative • The proposal narrative should provide the reviewers with a clear understanding of the organizations capacity to deliver services outlined in the request for proposal. • The narrative should be formatted using a 1” margin and no smaller than a 12 point font. Include a table of contents. The narrative must be typed and cannot exceed 24 single sided, double spaced pages. This does not include the budget and budget narrative, supporting documents, including the executive summary, table of contents, flow charts, letters of commitment and forms will not be counted as part of the 24 page limit. • Pages should be numbered and include a header and footer identifying the agencies name • Identify each section and subsection of the evaluation criteria to which you are responding: • Organizational Strength and Demonstrated Effectiveness • Program Components • Evaluation and Data • Staffing and Model Development • Cost Reasonableness and Competitiveness

Budget and Budget Narrative and Cost Allocation Plan • See Budget Template , Budget Instructions, and model Cost Allocation Plan can be found on COWIC Website http://www.cowic.org/vendor-library.php • A Budget Narrative is required to substantiate both revenue and expenses included in the budget. Budgets should reflect total cost identifying COWIC’s share and other payers documented by Letters of commitment, MOU’s or other grant award documentation • Evaluation Factors • Length and Intensity of program design • Staff to participant ratios • Support services • Leveraging of funding from other resources • Costs per participant • Cost proposed are necessary, reasonable and allowable per Federal Regulations. All cost must be allowable under WIA
