Early Years Systems Child with identified Complex Needs Child attending mainstream Nursery HI/VI/MSI Specialist Teachers Potentially involved from birth Referral from Health Identified as needing additional support Early Support intake Meeting STLS, DISCO or EY Specialist Teacher & Portage attend from Educations Child Doesn’t meet criteria Refer to Early Years LIFT • 3 per term per district • Suggested Attendees; • Educational Psychologist’ Equality & Inclusion Advisors, STLS, O&A Specialist Nursery Manager, Children’s Centre Rep, Setting SENCo’s, SEN District Lead, Portage Child meets criteria Joint Assessment (MMA) Enhanced Core Mainstream Placement With Support from LIFT Specialist Observation And Assessment Placement Keyworker Specialist Nurse Advisor Lead Professional Could be Portage Support: Equalities and Inclusion – Advice and Training STLS Intervention Sign Posting Training Collaborative Support and Guidance Individual Pupil Advice Augmented Funding • Team around the child (TAC) • TAC Plan created • When considering educational provision or if the child • has particularly complex needs affecting learning, • development and health that are likely to require a high • level of special educational provision, the TAC could • request the attendance SEN Area Statutory Assessment • and Placement Officer (Early Years) to consider initiating • an EHC Plan Support not enabling progress Setting to request Statutory Assessment Statutory Assessment Process
