Fidelity Support Services have goal-oriented nurses who assist individuals to overcome some of the challenges in achieving their goals and reach their full potential, by providing the necessary nursing care and support. This may involve working with the individual to develop a plan of care that is tailored to their specific needs, preferences, and goals, and connecting them with the appropriate resources and services

Support Coordination and Specialist Support Coordination Participants with the need for targeted goals can have a programme designed specifically for them. Our support coordinators and specialist support coordinators work closely with individuals and their families to identify their needs, develop a plan of support, and connect them with the appropriate services and resources. We also provide ongoing support and follow-ups to ensure that the supports and services provided to the individual are consistent and reliable over time. Programmes include but are not limited to; individuals accessing community resources and services, recreational programs, transportation services, employment opportunities among others.
