  5. The number of people with diabetes has risen from 108 million in 1980 to 422 million in 2014 • The global prevalence of diabetes among adults over 18 years of age has risen from 4.7% in 1980 to 8.5% in 2014 • In 2015, an estimated 1.6 million deaths were directly caused by diabetes (WHO Nov 2017)

  6. Foregut hypothesis Exclusion of duodenum and proximal jejunum from the transit of nutrients may interrupt signals that lead to insulin resistance and Type 2 DM

  7. Hindgut hypothesis • Enhanced delivery of nutrients to distal ileum alters secretion of hormones and improves glucose metabolism • Increased release of GLP-1 and PYY(Peptide YY) • -Anorectic • -Incretin effect -Triggers the ileal brake mechanism

  34. Conclusion • Metabolic surgery aims at treating metabolic derangements of obesity by alterations of GI anatomy • Metabolic surgery is significantly more effective than intensive medical therapy in treating T2DM related to obesity

Conclusion • Multiple parameters and scoring system can predict the change of DM remission • If ABCD score ≤3 or DiaRem score ≥8: DM remission rate < 10% • Metabolic surgery is recommended as an option to Type 2 diabetes in: • Class III obesity regardless of glycemic control or complexity of glucose lowering regimens • Class II obesity with inadequately controlled hyperglycemia despite lifestyle and optimal medical therapy.
