What is it? • Vascular risk assessment and management programme • Midlife MOT – checks whether some of the body’s most important systems are all running smoothly • NHS Health Check will assess the risk of developing heart disease, stroke, diabetes, high blood pressure , kidney disease and certain types of dementia and give personalised advice on how to reduce risk • Everyone is at risk of developing these conditions but these can often be prevented • It also aims to reduce alcohol related harm and raise awareness of dementia in those aged over 65

Who is it for? 40 – 74 year old adults who do not have • Atrial fibrillation • Coronary heart disease • Chronic kidney disease • Diabetes • Hypertension (raised blood pressure) • Hypercholesterolaemia (raised cholesterol) • Heart failure • Peripheral arterial disease • Stroke • Transient ischaemic attack • Patients who are prescribed statins • Previously been assessed and found to have a 20 % or more risk of developing cardiovascular disease in the next 10 years either through the NHS Health Check programme or any other health service in England

  6. Why is it important? National Context • Circulatory diseases are the second leading cause of premature mortality followed by stroke • Diabetes, kidney disease and stroke together, make up a difference of a third in life expectancy between the most deprived areas and rest of the country • Economic modelling by DH showed 1600 heart attacks & strokes & 650 premature deaths could be prevented and over 4500 cases of diabetes identified through health checks

  7. Why is it important? Local Context • Circulatory diseases including cardiovascular disease and stroke are the leading cause of preventable premature deaths in Redbridge. • High prevalence of CHD, diabetes, hypertension and CKD • Diabetes is projected to increase from 8.7 % to 11.5 % • Cranbrook / Loxford has the highest number of people living with diabetes & CHD in Redbridge

  8. How to get a NHS Health Check? • Adults aged between 40 and 74 who do not have any of the above mentioned conditions will be invited by their GP every five years for an NHS Health Check • Eligible patients can request their surgery for a health check

  9. What happens at the NHS Health Check? • Record age, gender, ethnicity, • History - past history, family history, lifestyle questions including smoking and alcohol intake • Examination - Height, weight, BMI and blood pressure measurement • Investigation - Blood tests for glucose and cholesterol • Results of above tests and an overall risk score for getting heart disease or stroke in the next 10 years • Discussion of results and appropriate referral to lifestyle services or management

Taking Action • Based on results patients will be categorised into high, medium or low risk • Personalised advice and support will be offered to stay healthy and lower risk if any of the results need improvement. • Patients identified as low or medium risk - Suggestions on changes to diet • Increasing exercise • Patients identified as high risk • Drugs to control blood pressure or cholesterol along with stopping smoking, losing weight and becoming more active • Patients diagnosed with long –term conditions will then leave the NHS Health Checks Programme and go on to the respective disease register

How can the NHS Health Check help? • Gives a good idea of the risk of developing vascular conditions • Some risk factors cannot be changed • A number of things can be done reduce risk such as stopping smoking, maintaining a healthy weight, cutting down on alcohol, increasing exercise and having a healthy, balanced diet • Provides information and support to enjoy a better quality of life • Long term condition diagnosis – better management of condition and preventing complications

Conclusion • Important programme to reduce cardiovascular morbidity and mortality • Aimed at adults between 40 and 74 without any pre-existing vascular condition • In Redbridge, number of health checks offered is below the national target and less than London and England • Uptake of NHS Health Checks is above target (78 %) in Redbridge • Promote the programme to those eligible
