Rank 1 – 8 (lowest to highest) • Lower or higher risk of nursing home placement • Rank 1 and 2 • Annual assessment • Rank 3, 4, 5, 6, 7, 8 • Semi-annual assessment • Rank 6,7,8 • 6: Imminent Risk • 7: Aging Out from DCF Services • 8: Adult Protective Services (APS) • Required to serve within 72 hours of receiving referral • Serve clients with the highest rank (highest need) first Ranking system / Prioritization

Encourage the provision of care for elders in family-type living arrangements in private homes • An alternative to nursing home placement • Provides a monthly stipend for caregivers for basic support and care of an elder individual • To help offset the cost of care giving, NOT to pay the caregiver Home Care for the Elderly (HCE)

Eligibility • Clients must be aged 60 or older • At risk of nursing home placement • Financially eligible • Based on income and assets • Caregivers must be at least 18 years of age • Capable of providing a family-type living arrangement • Willing to accept responsibility for the needs of the client Home Care for the Elderly (HCE)

  14. Services • Case Management • Case Aide • Basic Subsidy • Intended for support and health maintenance • Special Subsidy • Payments for goods and services necessary for health and well-being of elderly client Home Care for the Elderly

Passed by the Florida Legislature in 1973 • Designed to assist functionally impaired elders to live dignified and independent lives in their own homes • Eligibility • Individuals must be aged 60 or older • Functionally impaired • Determined to be at risk of institutionalization (nursing home placement) • Co-Pay* for services based on income *(See Co-Pay schedule) Community Care for the Elderly (CCE)

  16. Began with the passage of state legislation in 1985 Provide services and training to address the needs of individuals with Alzheimer’s disease and related memory disorders Research related to the cause Prevention Treatment and cure of the disease Alzheimer’s Disease Initiative (ADI)

Eligibility • Diagnosed or suspected of having memory loss where mental changes appear and interfere with the activities of daily living (ADLs) • 18 years or older • Not just Alzheimer’s disease • Must have a caregiver • Caregivers are eligible to receive training and support services to assist them in caring for the individual with memory problems • Co-pay* for services based on income *(See Co-Pay schedule) Alzheimer’s Disease Initiative (ADI) cont.

  20. Funds the federal Administration on Aging, State Units on Aging, and Area Agencies on Aging Organization and delivery of supportive services and nutrition programs for elders Must be bid on every three years Older Americans Act (OAA)

  21. Eligibility • Individuals aged 60 or older • Targeted toward low-income, minority elders • Elders are not required to pay for services • Donations are accepted • Volunteer recruitment is encouraged Older Americans Act (OAA) cont.

Part B (OA3B) • Access Services • Public Education • Speaking to groups or distributing materials to individuals at public gatherings about services and opportunities available • Outreach • Reaching target individuals • Community setting or in their home • Information • Responding to an inquiry from a consumer regarding public and private resources and available services • Referral • Information is obtained about a person’s needs and the person in directed to a particular resources (Note: Information and Referral services are provided by the Aging Resource Center) OAA Title III-Part B

Supportive Services • Homemaking • Housing Improvement • Chore • Companionship • Legal Services • AAA contracts with Legal Services for North Florida, Inc. • Legal Assistance protects against the loss of basic civil rights • Ensures the autonomy and independence of older persons • Client are not charged for services. OAA Title IIIB (OA3B)

Part C (OA3C) • Nutrition Services • Congregate Meals (referred to as C1) • Facility based • Socialization • Games/activities • Home Delivered Meals (referred to as C2) • Home bound clients • Nutrition Education • Safe food handling • Medications and food • Healthy eating habits • Nutrition Counseling • If funding is available OAA Title III-Part C

Eligibility • Clients must be aged 60 or older • Determined to be financially eligible by Department of Children and Families (DCF) • Determined to be functionally eligible • Through assignment of Level of Care (LOC) by DOEA/CARES (Comprehensive Assessment and Review for Long-Term Care Services) Aged and Disabled/Medicaid Waiver Program

  26. Services Include • In-home assistance (parallel to those of CCE) • Medical care and prescriptions • Durable medical supplies • Fair Hearing requests are filed with Department of Children and Families Aged and Disabled/Medicaid Waiver Program

Emergency Home Energy Assistance for the Elderly Program (EHEAP) • Eligibility • Applicant must be 60 years old or older • Verification of a home energy crisis • Household income must be below 150% of the poverty guidelines • Household may apply once per season • Cooling Season: April 1 through September 30 • Heating Season: October 1 through March 31 Additional Programs
