PHEP Certificate Program • Program Purpose • This certificate program is designed to increase the capacity of individual workers and governmental public health agencies in Illinois, and nationally, to prepare for and respond to all types of public health emergencies and disasters, whether naturally occurring or man-made.

PHEP Certificate Program • Program Candidates • Individuals working in or aspiring to positions in public health agencies that are engaged in preparing for and responding to a wide array of emergencies that have public health implications • Public health workers with a baccalaureate degree who may not be able to seek or complete a master’s degree program.

PHEP Certificate Program • Competency Framework • Based on a 22-competency framework required by the federal Pandemic and All Hazards Preparedness Act (PAHPA) legislation and developed for CDC by a national panel of experts convened by the Association of Schools of Public Health in 2011 • Students will learn: • The philosophy, purpose, history, organization, functions, tools, activities and results of modern public health practice • Basic principles of public health emergency preparedness and response activities. • Analytic skills and assessment tools used in public health emergency preparedness and response activities • Management tools and methods for public health emergency preparedness and response

Public Health Emergency Preparedness Certificate • 4 online courses (14 credits; 12-15 months) • CHSC 400 - Public Health Concepts and Practice (3 credits; Fall, Spring, Summer) • CHSC 460 - Introduction to Public Health Emergency Preparedness and Response (3 credits; Fall, Spring) • CHSC 461 - Public Health Emergency Preparedness and Response Assessment Methods (4 credits; Summer 2013, then Spring 2014 and every Spring) • CHSC 462 - Public Health Emergency Preparedness and Response Management (4 credits: Fall 2013 and every Fall) • Courses can be transferred into MPH degree program after acceptance

Application Requirements • Online application fee of $60 • Official Bachelor’s degree transcripts • Personal Statement: 300 to 500 words describing your interest in the program, your professional background, work experience and long term career goals (in MS Word or PDF file) • Current CV/Resume (in MS word or PDF file) • TOEFL Score (required for international applicants or domestic applicants with a degree from a foreign university where English is not an official language) • After submitting your application, send an e-mail to chscert@uic.edu to alert us that your application has been submitted online.
