  2. What is the Home Health Patient Tracking System (HHPTS)? The Home Healthcare Patient Tracking System is an electronic tool to be used by the Broome County Home Care Coalition members, Broome County Health Department (BCHD) and the Broome County Emergency Operations Center (BC/EOC)in the event the Broome County Emergency Manager declares an emergency for the area.

  3. What is the HHPTS? • A web-based software tracking system that includes patient demographic information including name, emergency contact information, New York State Department of Health Acuity level, and any special circumstances related to that patient’s care needs. • When it has been determined by the BC/EOC that an emergency exists, the EOC can access the HHPTS to provide efficient home care services to the patients in the agency. The agency can also access their information and respond accordingly. • The home care agency staff are also in the system and can be contacted to respond to patients nearest their residence, if needed.

Why do we have such a tool? • The Good: • The Broome County Health Department received a grant and worked with students from Binghamton University to develop the software for this tool. • The Broome County Home Care Coalition has worked collaboratively to develop the protocol for this tool. • The Bad: • It has taken the different organization’s attorneys more than 2 years to come up with a Memorandum of Understanding to cement the relationship.

Why do we have such a tool? • The Ugly: • The Broome County area has suffered several flooding events over the last several years. • The City of Binghamton suffered a violent massacre of innocent people at the Binghamton American Civic Association in 2009. • These events affect our patients, our associates, and our ability to care for both.

  6. Objectives of the HHPTS • Provide an efficient response in the event of an emergency in Broome County and mitigate or prevent any interruption in home care services provided • Collaborate with the BC/EOC to ensure all patients are cared for based on acuity level • The ability for staff who are nearest the patient to respond to provide care as needed, to reduce traffic and safeguard the safety of staff
