Open to U6 - U11. In consideration of the player’s participation in the soccer programs and activities of the USYSA Parties (“the Programs”), I, for myself and the player and our respective heirs, administrators and successors, intending to be legally bound, hereby release and indemnify the USYSA Parties, the owner and operators of the facilities used for the Programs, and their respective directors, officers, employees, agents and representatives from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player’s participation in the Programs including, without limitation, player’s transportation to/from any Program, which transportation is authorized. I further grant the USYSA Parties the right to use the player’s name, picture and/or likeness in printed, broadcast and other material concerning the Programs provided such use is related to the player’s status as a participant in the Programs.As the parent or legal guardian of the above named player, I hereby give consent for emergency Medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. 15. Email Confirmation (Academy) Dear Academy Family, Thank you for signing up for our Academy Winter Session. The first session starts Tuesday, January 21st from 5pm to 6pm. We have Academy sessions every Tuesday and Thursday until we finish on Thursday, February 27th. We're excited to get started and we look forward to working with all of your children.  A couple things to know to help you prepare for the Academy: 1) We're on field #1 at Snedigar. For field info go to http://www.santansoccer.com/facilities-fields/index_E.html 2) Check in starts at 4:45pm on Field #1. All new 1st time players will receive a new Adidas MLS replica Academy uniform. Returning players use your old uniform. All players are asked to wear their uniforms for every Academy session. 3) We'll have a short parent discussion with information about the Academy right after check in with time for Q & A. 4) Players can be dropped off and picked up after each session if needed. We encourage parents to stay and watch so when your kids go for water you'll be there or if they need anything during the session. For our very last session we'll have the parents come out on the fields with the players for some fun parent/player soccer games. Start getting yourself in shape now!!! 5) Players should wear soccer cleats, shin guards, and bring a ball. Size 3 ball. 6) Bring water for all the break times. We'll have a lot of fun! All the players will develop new soccer skills in a real fun environment.
