BEST OF THE BEST 2009 www.criticalcarenutrition.com PEP uP Protocol: Enhanced Protein-Energy Provision via the Enteral Route in Critically Ill Patients • More calories and protein for ICU patients for better clinical outcomes…… • Start feeds at goal rate in stable patients, not @ 25 mls/hr • Use a concentrated well-tolerated formula (Peptamen 1.5) • Order feeds by 24 hr volume, not by hourly rate • RN sets the feed rate, and catches-up missed volume • Start motility agents immediately, not waiting for high gastric residuals • Start BeneProtein supplement immediately • Trickle feeds in unstable patients • Increase threshold for gastric residuals to 250 mls • RN reports adequacy of nutrition intake on daily rounds Just say ‘NO’ to NPO!
