Community Health Care CHS MEDICAL PROGRAM PROGRAM IMPACT • Funded 34 tribal, urban Indian, and community health clinics • Services at 123 locations • 128,624 sliding fee medical patients • 337,706 sliding fee medical visits • Averaged 3 sliding fee visits per patient • Increased number of patients with medical homes

Mason Dental Clinic CHS DENTAL PROGRAM PROGRAM IMPACTS • 32 tribal, urban Indian, and community health clinics • 75 locations in Washington State • 58,435 sliding fee dental patients • 175,168 sliding fee dental visits • Increased outreach services • Oral health education • School sealant programs

Lower Columbia Head Start HEALTH LITERACY Building a better future for the most vulnerable children by providing parents with health care skills and knowledge • Program Objectives • Become better caregivers by improving knowledge and skills • Empower parent’s decision making • Enhance self esteem and confidence • Reduce healthcare costs • Provide a model for health literacy education programs CHS in collaboration with UCLA to implement the UCLA Health Literacy model program.
