Maine Demographics: A Profile • Maine is aging rapidly and at a rate faster than that of the rest of the nation. • National studies indicate elders are most likely to experience one or more chronic illnesses. • Five chronic diseases—heart disease, cancer, stroke, chronic lung disease, and diabetes— account for more than two-thirds of all deaths in the United States.

Maine Demographics: A Profile • Maine ranks fourth nationwide for deaths caused by chronic diseases • Healthcare costs are estimated at $1.5 billion a year for chronic disease groups. • Within this chronic disease group, cardiovascular disease is the leading cause of death among elders and accounts for over $437 million in hospital charges

HomeHealth Visiting Nurses (HHVN)Agency Profile • Service area over 2,000 square miles • In FY 2007, HHVN clinicians traveled over 1.6 million miles providing over 114,335 home health visits to 7,357 patients. • 32% (N 1,415) of HHVN’s 4,379 Medicare patients had one or more chronic diseases • 28% hospitalization rate
