    - 152 LA’s = 10 Excellent, 93 Good, 40 Adequate & 9 Poor

    - In 2008/09 = 173 SCR’s, 219 children & 113 deaths

    - weak management, lack of joint working, failure to focus on the needs of

    the child, insufficient staff expertise, not making adequate risk

    assessments, poor identification of ethnic or social cultural issues
