  3. Program Focus Reduce the leading causes of death including cardiovascular disease, stroke, diabetes and lower respiratory disease

  4. Program Focus Develop systems, environmental and policy changes to promote healthy behaviors The choices you make are shaped by the choices that you have

  5. Health Care Activities Health care provider education program Educate and train health care providers to properly identify, prevent and treat pediatric obesity Focus on Proper screening Decision making for treatment Health equity Health literacy Counseling tips Goal Setting (nutrition, physical activity, tobacco)

  6. Update • So far, over 35 health care providers have been trained in Ounce (3 trainings) • Two additional trainings scheduled • Post tests indicate that Ounce is having a positive impact on physician practice

  7. Schools Comprehensive Nutrition policy 100% Tobacco free school policy Safe Routes to School Farm to School

Update • East Cleveland SRTS: • Completed application for travel plan • Await engineering study • East Cleveland School Gardens: • Chambers and Superior Elementary Schools • Working on a garden curriculum training (Food Land and People) • Euclid’s FACET team: • Completed a wellness based action plan • Comprehensive Tobacco Policy • CATCH training (includes nutrition, physical activity, and tobacco) and curriculum pilot • Cross promote community gardening and the Euclid Farmers’ Market
