Was this a Tier I or Tier II defect? (copy of PR/R with final response should be included in supporting documentation)

Include graphics/drawings/pictures that will help others understand the nature of the problem.

What was the approximate cost to your company for this spill? Use cost of quality worksheet as a guide line for cost element.
