  4. 2011-2012: Discover Health • New brand/logo was developed to increase employee awareness of wellness platform. • On-site biometric screenings were conducted with 9,399 employees. • 89% (18,557) completed the health assessment. • There were three options for Wellness Engagement.

  5. 2012-2013: Challenge Yourself • Over 500 employees walked through downtown to the GRB for the Wellness Fair on July 19, 2012. • Over 2,000 employees attended the Health Fair and received their biometric screenings. • 90% (17,797) completed the health assessment. • Wellness engagement options were expanded.

  6. 2013-2014: Improve! • Wellness has evolved and employees are actively engaged in wellness programming, now it’s time to Improve! • The 2013-2014 Wellness program will be geared towards incentivizing employees’ participation in wellness activities.

  7. 2013-2014 Wellness Incentive Plan Active employees on the city’s medical plan are eligible for a premium discount in May 2014. Three Step Process • Obtain biometric information. • Complete the online Health Assessment. • Earn at least 500 wellness points.

Wellness Champions-Zero Health Risk • Must take the online Health Assessment to determine risk. • Employees with zero risk based on Cigna’s risk stratification tool will not be required to fulfill the 500-point requirement and will automatically receive the benefit premium discount. They are strongly encouraged to participate in additional wellness activities, and remain eligible for prize drawings. • Cigna will notify employees in the zero risk category of their status through a personalized letter and an automated phone call.

Prize Levels per Total Points Accrued per Benefit Plan Year • Bronze: 505-650 points • In addition to a Wellness Champion rubber wristband or an apple pin, individuals in this category will be entered into a drawing for the prizes listed below: • $25 gift card to healthy food store/restaurant/fitness store (300) • Insulated Lunch Bag (300) • 1-week Weight Watchers Pass (300) • Silver: 655-800 points • In addition to a Wellness Champion t-shirt, individuals in this category will be entered into a drawing for one of the prizes listed below: • $50 gift card to healthy food store/restaurant/fitness store (200) • 1-month Weight Watchers Pass (200) • 1-hour Massage Certificate (200)

Prize Levels per Total Points Accrued per Benefit Plan Year Cont. • Gold: 805-950 points • In addition to a Wellness Champion t-shirt, individuals in this category will be entered into a drawing for one of the prizes listed below: • $75 gift card to healthy food store/restaurant/fitness store (100) • 17-week Weight Watchers Pass (100) • 3-month YMCA Pass (100) • Platinum: 955 or more points • In addition to a Wellness Champion t-shirt, individuals in this category will be entered into a drawing for one of the prizes listed below: • Wii Game Console plus 1 fitness game (75) • $100 gift card to a healthy food store/restaurant/fitness store (75) • 6-month YMCA Pass (75)

  15. Summary Points • Our new wellness program will commence May 1, 2013. • All employees will be encouraged to obtain biometric numbers and complete the annual health assessment. (Voluntary Program) • Employees with zero riskbased on the 2013 health assessment will be exempt from additional wellness activities, but they are still encouraged to participate. • To be eligible for a premium discount in May 2014, employees with low, moderate or high risk factors will have to: • obtain their biometric numbers, • complete the annual health assessment, and • earn at least 500 wellness points through thirteen (13) options.
