  2. Mission: Rooted in Judeo-Christian values, our mission is high quality, compassionate, cost-effective health care for the communities we serve. Vision: Optimal health and well-being for our patients, our communities and ourselves.

  3. Phase 1: Mental Health • Work Process: • Start: by Jan 30th, 2014 • Time: 8 hours/day- 6 days/week • Patients: • ~10 new patients/month, ~30-40 active Pt.'s at a time. • ROI: • Reducing readmissions • Freeing up in patient beds

  4. Phase 2: Maplewood, MN • Work Process: • Start: by May, 2014 • Time: 8 hours/day- 6 days/week • Patients: • Not eligible for other outreach programs, (home care, CHF care, diabetes outreach) • ~20 active Pts./week • ROI: • Increased compliance with care plans • Improved utilization patterns • Decreased geriatric fractures, frequency of lift assists

  5. Phase 3: Hospital & Clinics • Work Plan • Start: by Sept, 2014 • Time: 8 hours/day- 6 days/week • Patients: • Not eligible for other outreach programs, (home care, CHF care, diabetes outreach) • ~80 active Pts./week • ROI: • Increased compliance with care plans • Improved utilization patterns • Decreased geriatric fractures, frequency of lift assists

  6. Summary • HealthEast System Integration by 2015 • 8 hours/day- 6 days/week • >100 active patients • Increased compliance with care plans • Increase quality and safety • Decrease readmissions
