  1. CHILD WELFARETARGETED CASE MANAGEMENT

  2. Child Welfare Targeted Case Management services are activities that coordinate social services and other needed services for eligible children and their families.

  3. Child Welfare Targeted Case Management (CW-TCM) activities are MA reimbursable for certified counties, certified tribes and private vendors under contract to counties or tribes if all eligibility and documentation requirements are met.

  4. Including: mental health services, habilitative services, educational services, health services, vocational services, recreational services, and related services including, but not limited to, the areas of volunteer services, advocacy, transportation, and legal services. The case manager helps the family access needed services:

  5. Case Management includes: • Assisting the child and the child’s family to access needed services • Coordinating needed services with other agencies • Insuring continuity of care • Monitoring and evaluating delivery, appropriateness and effectiveness of services

  6. The Record Review • Why have we been conducting a record review? • How have counties been chosen for the review? • Will all counties’ records be reviewed? • How is the review being conducted? • What are we finding in the review?

CLIENT ELIGIBILITY • WHO IS ELIGIBILE FOR CW-TCM SERVICES? • MA or MinnesotaCare recipients who are under age 21 and who meet one of the following conditions: • at risk of placement or in placement as defined in MN Statutes, section 260C.212, subd.1 • at risk of maltreatment or experiencing maltreatment as defined in MN Statutes, section 626.556, subd. 10e or • in need of protection or services as defined in MN Statutes, section 260C.007, subd. 6

  8. Documenting Eligibility • Which eligibility criteria does the child meet? • Briefly describe the reason the child meets this criteria • Is the child enrolled in the Medical Assistance Program or MinnesotaCare?

  9. CW-TCM Finding • Johnny is in need of protection or services as defined in MN Statutes § 260C.007, subd. 1, due to……or because ………. • Child welfare targeted case management services are needed and will be provided.

  12. Dual Case Management Rationale and Roles • Questions to be answered: • Why are services of two case managers necessary? • What services will each case manager provide? • Which case manager will be considered primary? • This can be part of the case plan

Case Plan • A case plan needs to be developed before claims may be submitted. • A separate written CW-TCM plan is not necessary as long as CW-TCM activities are identified in the case plan to be used. • The case plan needs to address the individual child’s needs. • The case plan is a living document

  14. Case Plan Formats in SSIS • Child Protection Plan • Family Service Plan • Out-of-Home Placement Plan Any of these may be used and identified as also being the CW-TCM Service Plan Case plans should be signed by all concerned

  15. Collateral/Relationship Lists • Include everyone who is a possible contact on the child’s behalf. • Can be a part of the case plan or a separate list • Eliminates need to mention relationship each time a contact is made.

  16. DOCUMENTATION OF CONTACTS • Payment for CW-TCM may be claimed for a face-to face contact with: • the client, • the client's family, • primary caregiver, • legal representative, • or other relevant person named in the case plan

  17. CONTACTS continued…. • Contact must be identified as necessary to the development or implementation of the goals of the child’s case plan • Contact must be regarding the status of the client, the individual service plan, or the goals for the client.

  18. TELEPHONE CONTACTS • Reimbursement for CW-TCM may be claimed for a telephone contact if: • the child is placed outside of the county or the reservation in foster care, or placed through the Interstate Compact • placement is more than 60 miles beyond the county or reservation boundaries, • there is at least one contact per month and not more than two consecutive months without a face-to-face contact.

  19. Documentation of Contacts • client name • date of contact • type of contact (face to face or telephone) • person contacted and relationship to the client • location of the contact • person making the contact • The nature and extent of services provided

SSIS DOCUMNETATION • To generate a CW-TCM claim staff must document the contact accurately in SSIS. • The service and activity selected on the contact screen must have a BRASS code and related HCPC that is potentially billable for CW-TCM • The contact note must reflect case management activity.

For each child for whom CW-TCM is claimed the case manager must: • determine the child meets the eligibility criteria • determine the child’s need for service, • document the eligibility and CW-TCM finding, • prepare a written service plan, • keep a written record of contacts with the client or others, • review eligibility criteria and service plan annually • All of the above must be documented for each child in the case file.
