  4. Peer Run Drop In • 7 day per week drop in • Clients can come in for unstructured time • Grant funded for people without Medicaid • Opportunity for engagement and work with homeless populations

  5. High Intensity Outreach • 60/70% service off site • Motivational Interviewing • Harm Reduction • 24 hour crisis line for clients and landlords • Landlord outreach • SOAR

  6. Integrated Healthcare • Onsite primary Healthcare via FQHC collaboration • Nursing staff onsite and on embedded in outreach teams • Finding the right partners • Continuity of care at point of re-entry/Inpatient/Detox/ER • Health Education and coaching

  7. Housing Options • Temp housing at release SRO/Shelter • Site based options • Independent apartments
