Operationalevents at a nuclearfacilitymayconsist of single transientsorobservations as well as recurringor common causefailures, humanerrors and omissions. Based of the reportsprovidedby the operating organisation and itsownobservations made duringinspections, STUK mayappoint an investigationteam as appropriate to assess a specificevent. The specificduty of such a team is to determine the rootcauses of the event and defineobjectives for correctivemeasures.

Written instructions shall be provided for process-related procedures and the manner of carrying out the activities. The possibility of human error in work performances shall be taken into account when defining the processes and the activities contained in them. The processes shall be planned so as to identify and disclose potential errors as early in the process as possible.

Whenplanning the tasks of the shiftteams, careshallbetaken to ensurethat the workload, participation in training, and the humanfactorsrelated to the duration of the shifts, shiftrotation, and restperiodsdonotaffect the teams´ability to performsafety-relatedwork. 717. The procedures and guidelinesshallbesystematicallyvalidated and verified. Validationshalladdress the role of humanfactors in the procedures. The validation of the procedures and guidelinesshallbebased on simulationsorothersuitablemethods, primarilybyusing a trainingsimulator.

The PRA shallutilise operating experience feedback collectedfromsimilarplantsorcorrespondingapplications. The licenseeshallmaintain a database of the reliability of safety-relatedcomponents as well as initatingevents and humanerrors.

The licensee shall carry out a generic investigation of multiple events when similarities can be seen with regard to their underlying technical causes, organisational or human factors, or other recurrence problems.
