Project Goals • To assist in the development and implementation of a new treatment modality in the continuum of care at Psychiatric Institute of Washington (PIW) • To develop a culturally sensitive program • Integrates evidence based approaches to treating substance use disorders • Enhance community presence and visibility for New Directions Intensive Outpatient Services

  3. Inception of Program Response to a growing trend for increased capacity for outpatient treatment for individuals that are impaired with substance use disorder

  4. Target Population • Adults who are employed and insured • Adults who meet intensive outpatient substance criteria

  5. Key Collaborations • Metro Employee Assistance Program (EAP) • Arlington County EAP • United States Postal System EAP • Fannie Mae EAP • Fairfax County EAP • AMTRAK EAP

Outreach Activities • Conducted weekly meetings with PIW outreach team • Discussed contacts made with referral sources to enhance census. • Compiled list of all Employee Assistance Programs (EAP) public and private sector providers • In collaboration with PIW team designed, developed, developed and disseminated brochure about New Direction Programs • Conducted weekly meetings with EAP’s referral sources, therapists, physicians, other treatment agencies • Team participated in quarterly EAP conference • Web Link established on PIW website – highlighting New Direction Activities • On 29 September 2006 – Conducted a open house showcasing program – 30 people attended

Overview of New Directions • New direction is a unique intensive outpatient program operating under the auspices of PIW • The program utilizes traditional interventions and evidence methodologies • The intensive outpatient program is one component of an array of services in PIW’s continuum of care

  8. Clinical Criteria • A maladaptive pattern of substance use manifested by recurrent and significant adverse consequences related to the repeated use of substances. • Sometimes used interchangeably with the term substance dependence • Diagnosis indicated in a DSM-V

  9. Interventions Used • Motivational Interviewing, Stages of Change ~Precontemplation ~Contemplation ~Preparation ~Action ~Maintanence ~Recurrence • Psychosocial Assesment • Group Therapy • Cognitive Behavioral Therapy • Individual Therapy • Urinanalysis Testing

  10. Benefits of Motivational Interviewing • Easily applied in a managed care setting, where cost containment issues are of great concern.

  11. Challenges of Program Implementation • Changing philosophies of employers • Program staff recruitment and retention issues • Poor utilization of services • Patient noncompliance, no-shows, postive UA’s, etc,
