  2. Lesson 11 Objectives • Conduct a closeout briefing and review with team members. • Conduct closeout briefings with agency representatives.

  3. Team Closeout Briefing • Determine remaining tasks to be accomplished. • Assign follow-up task assignments to team members. • Determine who will maintain evidence (until final report is completed).

  4. Team Closeout Briefing • Cleaning and turning in equipment. • Collection and destruction of all working papers and draft copies of reports.

  5. Team Review As part of the closure for the Serious Accident Investigation Team it’s important to review and critique the team’s performance. This process is an opportunity for the team members to improve their skills and abilities for future assignments. This feedback is also valuable to the agency to make improvements in Serious Accident Investigation Training and processes.

  6. Team Review • The Team Leader facilitates the briefing ensuring all members participate, keeping the team focused on the issues. The review should include: • Critique the accident investigation process used. • Discussion of what was planned. • What actually happened? • And what can be done in future assignments to improve process and performance of the team. • In general discuss: what was planned, what actually happened, why it happened, and what can we do next time to improve. • The Team Leader should send suggestions for process improvement to the respective Agency’s National Safety Office and/or the Fire & Aviation Offices.

  7. Team Review • The Team Leader and Chief Investigator should evaluate and make recommendations for team members to receive critical incident stress debriefing (CISD) for team members.

  8. Closeout Briefings with Internal and External Entities Closeout briefings may occur with any one of the following: • Agency Administrator • Agency Director • Fire & Aviation Directors • Agency Safety Office • Designated Safety and Health Official (DASHO) Handout 11-2

  9. Closeout Briefings with Internal and External Entities Closeout briefings may occur with any one of the following: (continued) • Cooperators • Local Authorities • Family Members • Accident Review Boards • NPS Board of Reviews • Congressional Staffers

  10. On-Site Closeout Briefings with Management Handout 11-2 The out-brief should cover: • Where SAIT is at in the process and expected timeline in complete process/final report • If an extension is requested beyond the 45 days, the Delegating Official(s) should ensure the affected agency administrator(s) are notified. • The known facts of the accident (e.g. timeline of the accident, cause of injuries, etc.) • Any findings which if left uncorrected could lead to future accidents • Any recommendations that can be implemented currently to prevent future accidents • Any evidence in local storage that is being retained at that location. • Release of the accident site back to the local unit • Disciplinary actions should not be discussed at all. Remember the objective of the investigation is Accident Prevention!

  11. Post Final Report Closeout Briefings Upon completion of the draft and final reports: • The Team Leader and/or Chief Investigator may be requested by the Agency to make a oral presentations to management, Accident Review Board/Board of Review. • Upon a request by OSHA a formal presentation may be needed detailing the factual findings of the investigation.

Post Final Report Closeout Briefings Upon completion of the draft and final reports: (continued) • The Agency Director may be requested to personally brief the Secretary of Interior/Agriculture to explain the accident and the corrective actions. • The Team Leader and/or Chief Investigator may be requested by the Agency to present the Factual Report to the immediate family.
