  2. Agenda • May 2005 In-house Laser Safety Audit Results • Upcoming Laser Safety Audit: Oak Ridge Site Office Sept 13-14, 2005

Jlab In-House Safety Assessment May 2005 • Focus: Agenda provided by DOE: • Establish clear roles, responsibility, and authority for laser operations • Demonstrate active oversight of all aspects of laser operations • Monitoring and evaluating mentors and LSSs that oversee students • Establishing and clearly explaining personal accountability, including reprimand policy for employees that do not meet safety performance specifications

  4. Findings of Internal Assessment • Based on: • agenda provided by DOE • ANSI standard for the safe use of Lasers • 9 Findings

  5. Findings of Jlab Internal AssessmentFindings 1-4: Policy Changes to EHS Manual • LSO approves wording on laser signage • Formally designate the deputy LSO: John Kelly • Formal designation and documentation of LSS quals • Formalize procurement process for purchase of lasers

  6. Findings 5-9 concern Training • Laser safety orientation: slide on DOE eye injuries • LSS’s review DOE injuries and root causes with all users • Evaluate and implement refresher training where necessary (LSO) • Formalize laser specific training, must indicate depth and scope of on the job training

  7. DOE Laser Audit • September 13th -14th • A list of LSS’s and users will be provided • Auditors will want to pick randomly from the list to meet with LSS’s and operators

  8. Successful Audit depends on • A complete hazard analysis of the laser system is documented (LSOP) • Line management involvement and LSO oversight is happening. • Staff demonstrate personal responsibility for working safe: They are aware of procedure details and wear eyewear and know to report those who do not. • Good housekeeping

  9. Roundtable Discussion • Formalize procurement process for purchase of lasers • LSS’s review DOE injuries and root causes with all users • Formalize laser specific training, must indicate depth and scope of on the job training (students)
