Project leads have been appointed in each area
A quality improvement steering group monitors the milestones for the project plan and reports to the Western Health Best Care – Coordinated Care sub committee.
Gap analysis for all Specialist outpatient clinics has been completed
Action Plans will now be developed for the target process not being currently met
Referral reports are available within Western Health reporting systems
Additional process – requires additional or reconfiguration of the same administration FTE
Western Health needs changes to the dictation software to be able to report on targets for communication (10.3 and 11.2)
