Materials Inventory Process • Retain original shipping boxes. • District boxes are white; school boxes are brown. • Verify boxes against the School Box Range Sheet. • Review packing lists and verify security ranges against the security checklist. • Record ANY missing materials.Note on the security checklist. • Immediately notify DRC of any discrepancies. Additional Materials • Carefully assess your need for additional materials. • Consider whether materials can be reused before ordering additional materials. • Materials such as Oral Administration Scripts, CD-ROMs, Braille CDs, and sign language DVDs may be reused. • Use the Intra-District Transfer Form to transfer materials between schools in your district. • Overage will not be sent to districts. This form may be completed and e-mailed to the SC Project Team. • Orders received before 4:00 p.m. Eastern time will be processed on the same day and shipped via overnight delivery. • Additional materials may be ordered between • June 12 • and July 16. Form A Test Materials • Test Booklet • Oral Administration Script or Audio CD-ROM • ELA and Mathematics • Students must answer in their answer document (unless their IEPs state otherwise). • Use with any Form A ELA or mathematics test booklets when oral administration is specified in a student’s IEP. • Use when the testing of an LEP student requires an oral administration of the mathematics test. • Audio CD-ROMs are PC-compatible and are designed for use with a computer only.

  31. Labeling – Answer Documents Override Linking Labels • Override Linking Labels mustbe used for all students. • Each sheet contains five pairs of white Override Linking Labels,side-by-side. Complete all information on each label. • Hand-coding the student’s name on the answer document is mandatory. If the student’s name is not hand-coded, the student will be reported as “No Name Available.” • Grid all applicable demographic information on the student’s answer document. TAM Appendix A
