Procedures: Before and After • C&G officer requests disclosure forms (700U or FFD). • If the disclosure form is positive, C&G officer requests the addendum form. • All forms are routed to the COI staff for further processing and COIOC review. • C&G officer requests disclosure forms (700U or FFD). • If the disclosure form is positive, COI Analyst requests longer addendum form. • Form is processed by the COI Analyst and reviewed by the COIOC.
