BEFORE SUBMITTING REQUESTS • Speak with your professor • If your professor is able to accommodate you for your exam, there is no need to submit a form for that class. • If you have a conflict caused by extended time on examinations, discuss alternate times • Requested dates and times should be exactly the same as your class unless otherwise discussed AND agreed upon with your professor. • Any date/time that differs from the professor’s schedule requires professor approval.

  5. ACCESSING REQUESTS • The direct link to the application is disabilitycenter.missouri.edu/exams • Log in using your pawprint (i.e. tigert) and the same password used to log into myZou and University webmail. • Click “Log In”

POLICIES & PROCEDURES • On the following page are the Policies and Procedures of our office for scheduling and administering accommodated exams. • If “Deny” is selected, you will receive the following message and be unable to access any more of the application until “Accept” is selected. • If “Accept” is selected, you will be taken to the next page to update your contact information. • You will be asked to read and accept the policies and procedures once every three (3) months.

EDIT YOUR ACCOUNT • Following your agreement to the policies and procedures, you will be directed to the “Edit Your Account” page to update your phone number and email preferences by using the drop down options. • Review of the Policies and Procedures page can be done at any time by selecting the “Policies and Procedures” tab at the top of the page. • You can also view all of the times you have accepted or denied the policies and procedures on this page.

SCHEDULING • This feature allows you to submit dates for your courses. • This may be beneficial if: • You choose to submit dates/times established by your instructor. • Your instructor changed a requested date/time and you need to make an update. • You need to submit an alternate date/time for a course due to scheduling conflicts or certain accommodations. These alterations must be approved by your instructor.

SCHEDULING • If an instructor has rescheduled the date/time of an exam and the new date does not provide nine (9) days’ notice to the Disability Center, • Email DisabilityCenterExams@missouri.eduand copy your instructor to the email notifying us of the date change as soon as possible. • Modification requests can be denied at the discretion of the Exams Coordinator due to lack of notice and insufficient availability of exam locations and proctors.

SCHEDULING – Denying Requests • Requests will be denied if: • 9 days’ notice is not provided • Requested on weekend or at an unapproved time • First exams administered start at 8:00am • Last exams administered start at 8:00pm • Finals Week exams begin at: • 7:30am, 10:00am, 12:30pm, 3:00pm, 5:30pm, 8:00 pm ONLY • At such time, you will need to take your exam with class.

  21. SCHEDULING Select “Edit” If an instructor has rescheduled the date/time of a request, you need an alternate time due to scheduling conflict or need an alternate time due to an accommodation, select “Edit”. This will allow you to edit and schedule the examination selected.

  27. SCHEDULING MADE_UP 1000 - 01 INTRO TO MIZZOU Select “Locked” If “Locked” is to the right of a date/time, it means the deadline has passed to request the specific date. You will then be expected to either make arrangements with the instructor or take your exam with your classmates.

DAY OF SCHEDULED REQUESTS • 9 Days’ notice is required for all regular (non-finals’ week) requests. • Finals’ Week requests - due 3 weeks prior to start of Finals’ Week. • Arrive on time (if not 10-15 minutes early). • If you are 5 minutes or more late, you must take your exam with your instructor.

  41. DAY OF SCHEDUELD REQUESTS • All needs must be taken care of prior to the scheduled start time • Once your exam has started, you will NOT be permitted to leave the exam site. • Bring photo ID. • If no ID, your exam will not be administered. • At such time, you must take your exam with your instructor.

  42. DAY OF SCHEDULED REQUESTS • No Shows • Do not arrive for exam + No communication = No Show • Receive email reminding you of scheduled exam with our office & of the ODS “no show” policy. • 2nd “No Show” for the same course • Dropped from scheduled for that course for remainder of scheduled requests for current term

  43. DAY OF SCHEDULED REQUEST • Academic Integrity violations will be reported to instructors. • All cell phones, notes and other non-exam related materials should be left in your backpack. • To keep track of time, be sure to bring a watch. • Not all classrooms on campus have functioning clocks.
