  • Developed in 2000 (Thanks, Nurit!)

  • Acts as the “Front Door” to obtaining prenatal care and services, along with OHP. Patient Navigation/Case management services. 


OMC Services

  • OHP application assistance / Expedited OHP approval

  • Prenatal needs assessment & referral to PNC  

  • Referral for maternity case management/home visiting services, WIC, Dental care, and other prenatal services as needed

OMC tracking forms

Each site sends a one-page tracking form to the state OMC office for each client.

  • Problems:

    • Required faxing and maintaining reams of paper documents

    • All information had to be manually input from paper forms into a database

    • Required OMC staff to call/email local sites to get any missing information

  • Reduced staff time on data entry

  • Staff can focus on QA, troubleshooting, fielding questions

  • Local sites more accountable for their own data
