  • Acts as the “Front Door” to obtaining prenatal care and services, along with OHP. Patient Navigation/Case management services.

Each site sends a one-page tracking form to the state OMC office for each client.

  • Problems:

    • Required faxing and maintaining reams of paper documents

    • All information had to be manually input from paper forms into a database

    • Required OMC staff to call/email local sites to get any missing information

  • Clients served in 2011 4,279 hospitals and clinics

  • Fax sheets 8,558

  • Reams of paper at 500 sheets per ream 17

  • Number of reams per tree 16.67
