Benefit Timeframes Morton County Human Resources • Team Members can make annual adjustments: • Starting on October 27, 2014; & Ending on November 7, 2014 • All adjustments must be received by Morton County on or before November 7, 2014. • Important Note: We will be unable to grant any exceptions to this timeframe. • All adjustments made will become effective on January 1, 2015. • Any change in premiums will occur on: • For voluntary benefits (health, dental, vision, voluntary products, etc…), changes will occur on the December 2014 payroll. These programs are paid one month in advance. • For other programs such as retirement and flexible spending, those changes will occur on the January 2015 payroll as these programs are paid current. • The USAble Program will be cancelled. If you are interested in continuing a voluntary program, you can re-enroll through AFLAC. • If you are interested in Flexible Spending, you must re-enroll in this program annually.

Eligibility & Enrollment Morton County Human Resources • Upon enrollment, benefits will remain in force until the following plan year, unless you encounter a Qualified Life Event. • Team Member Eligibility: • A Team Member has the ability to enroll in the Morton County Benefit Program if they work at least 20 hours per week on a consistent basis. • Dependent Eligibility: • A Team Member may add a legally married spouse and or legal dependent (natural-born, adopted or legal guardian) through the age of twenty-six (26). • Enrollment Timeframe: • A Team Member may enroll at time of hire, during Open Enrollment or if they encounter a Qualified Life Event (QLE). • Team Members will have thirty (30) days from the date of hire or from the QLE to apply. • Team Members may also enroll during the annual enrollment window. • Qualified Life Event: • A Qualified Life Event is a mid-year event which requires an immediate change to your benefit elections such as a marriage, divorce, birth, death, loss or gain of other coverage, etc…

How to Enroll Morton County Human Resources • North Dakota Public Employees Retirement System (NDPERS): NDPERS Online System • www.nd.gov/ndpers • Employees with previously activated accounts, use your prior logon information. • New Employees must create an online account prior to the submission of new information. • (All paper applications will be made available online at the Morton County website, under Departments/Human Resources/Employee Benefits. • PEP Contributions: Please contact Kathy Krance or complete the PEP Form online and return to Human Resources. • Dental: Please complete a paper application available online. • Vision: Please complete a paper application available online. • Flexible Spending: Please complete a paper application available online. • AFLAC: Must meet with AFLAC Representative in person. • UNUM Life: Please complete a paper application available online.

Monthly Costs: Employee Employer Single - $50.51 $454.55 Family - $244.04 $976.18 For In-Network Services, here is a general summary of coverages: Annual Deductible: $400.00 Annually Per Person / $1,200 Per Family Co-Insurance Generally 20% of Allowable Charge Out of Pocket Costs: $1,150 Annually Per Person / $2,700 Per Family Co-Pays: $25.00 Per Visit Emergency Care: $50.00 Per Visit + 20% Co-insurance Prescription Medication: $5.00 Generic / $20 Preferred / $25 Non-Formulary Durable Medical Equipment: 80% of Allowable Charge Maternity Services: 80% of Allowable Charge

  9. Delta Dental – Dental Insurance Morton County Human Resources Delta Dental is the current provider for Morton County’s Dental Insurance program. This is a PPO provider with an extensive network of dentists in our immediate area. Monthly Costs: Single $30.09 + Spouse $61.39 +Child(ren) $66.47 Family $101 For In-Network Services, here is a general summary of coverages: Lifetime Deductible: $100.00 Annual Maximum: $1,000 Per Person Diagnostic and Preventive Services: 100% Basic Services: 80% Basic Oral Surgery: 55% Major Services: 55% Orthodontia: N/A

  10. Avesis – Vision Insurance Morton County Human Resources Avesis is the current provider for Morton County’s Vision Insurance program. This is a PPO provider with an extensive network of vision providers in our immediate area. Monthly Costs: Single $8.45 + One $14.79 Family $21.97 For In-Network Services, here is a general summary of coverages: Co Payment: $10.00 for Exam and $10.00 for Material Annual Exam: 100% After Co Pay Frames and Lenses: $200.00 (Lenses Annually/Frames Every Two Years) Contact Lenses: $130.00 Annually (In Lieu of Frames and Lenses) LASIK: $150 One Time Allowance

  11. UNUM – Life Insurance Morton County Human Resources • Morton County offers a Supplemental Life Insurance Program through UNUM. This program allows you to purchase additional life insurance over and above the current employer offering of $10,000 Life / $10,000 Accidental Death & Dismemberment. • Employees may elect up to five (5) times their annual salary in increments of $10,000. • The maximum allowance is $500,000. • An employee’s spouse may elect up to the employee’s election in increments of $5,000. • An employee’s dependent child may elect up to the employee’s election in increments of $2,000, not to exceed $10,000. • In order to elect spousal or dependent child life insurance, the employee must elect life insurance. • At the age of 65, the life insurance benefit will reduce by 65% of the original amount. • At the age of 70, the life insurance benefit will reduce by 50% of the original amount. • For rates, please see the 2015 Benefits Guide on the Morton County Website.

Discovery Benefits – FSA Morton County Human Resources • A Flexible Spending Account is a tool that allows employees to pay for un-reimbursable medical and daycare expenses with money that is considered pre-tax. • Because this money is taken out of your pay, prior to taxes, you are able to save money by reducing your taxable income. • The Annual Maximum for the Medical FSA is $2,500. • The Annual Maximum for the Dependent Care FSA is $5,000. • The Medical FSA Account can be used for CoPays, Coinsurance, Prescription Costs, Durable Medical Equipmentand other Out-of-Pocket Expenses not covered by Health Insurance. • The Dependent FSA Account can be used for Day Care for children (up to age 13). It will also provide Adult Day Care for any Disabled Dependent to include a Spouse.

AFLAC – Group Critical Illness Morton County Human Resources • If Morton County identifies enough interest (at least 25 individuals), we will begin to offer a Group Critical Illness program. • This program provides a lump sum benefit upon the diagnosis of not only one covered illness, but for each covered illness. This cash benefit is intended to cover out-of-pocket medical expenses and the costs associated with life-changes following a covered critical illness. • Plan Features: • Lump-Sum Benefits • Guaranteed Issue / Amount Equal to $10,000 • Spousal Coverage Available • Dependent Child Covered at 50% of Primary Insurance Amount at No Charge • Benefit Available @ $5,000 - $50,000 /$25,000 for Spousal Coverage. • Annual Health Screening Benefits Available • Portability Options • Cost Based on Age @ Time of Application / No Increase due to Age/Health/Claim • Immediate Effective Date

AFLAC – Short Term Disability Morton County Human Resources Short-Term Disability is an income replacement program intended to supplement your income in the event that a disability prevents your ability to earn an income. Monthly Benefit: $500 - $6,000 (subject to income requirements) Total Disability Periods: 3, 6, 12, 18 or 24 months Partial Disability Periods: 3 months Costs will vary based on elimination periods and elected monthly benefits. AFLAC – Accident Program Morton County Human Resources • In the event of an accident, AFLAC’s Accident Indemnity Advantage program pays benefits of cash directly to you. • This program is intended to provide financial resources to help with expenses incurred due to an injury, assist with ongoing living expenses or to help with any purpose you choose. • The Accident Indemnity Advantage Offers: • No Deductibles or Co-Pays • No Lifetime Limit • No Network Restrictions • No Coordination of Benefits

AFLAC – Cancer Care Morton County Human Resources • The AFLAC Cancer Care Plan provides individuals and families to cope financially and emotionally if a positive diagnosis of cancer ever occurs. • The benefit amounts vary based on services. However, benefits are available for: • Cancer Wellness • Cancer Diagnosis • Cancer Treatment • Hospitalization • Continuing Care • Ambulance, Transportation, Lodging and Other Benefits

AFLAC – Critical Care & Recovery Morton County Human Resources Aflac’s Critical Care and Recovery Program provides financial lump sum cash payments in the event you suffer from a significant life event that is medically related. First-Occurrence: $5,000 Named Insured/Spouse; $7,500 Dependent Children Re-Occurrence: $2,500 Secondary Specified Health Event: $250 Hospital Confinement: $300.00 Per Day Hospital IC for Sickness: Days 1-7 / $700 Per Day // Days 8-15 $1,200 Per Day Hospital IC for Injury: Days 1-7 / $800 Per Day // Days 8-15 $1,300 Per Day Major Organ Transplant: $25,000 Continuing Care: $125 Daily up to 75 Days Ambulance: $250 Ground / $2,000 Air Transportation: $0.50 Per Mile up to $1,500 Per Occurrence Lodging: $75.00 Per Day

AFLAC – Hospital Confinement Morton County Human Resources • Aflac’s Hospital Confinement program will provide benefits when a covered person incurs a charge for confinement in a hospital. • Aflac Hospital Advantage offers benefits for: • Hospital Confinement • Rehabilitation Facility • Emergency Room • Short-Stay • Physician Visit • Medical Diagnostic and Imaging • Ambulance • Surgical • Invasive Diagnostic Exams

  19. Employee Assistance Program – EAP Morton County Human Resources Morton County offers an EAP program that provides employees with professional support when complex situations arise. When personal and family problems occur, you and your family have the option of reaching out to St. Alexius for a free confidential assessment, counseling or a referral! This is a confidential program where no information is shared with the employer. Your confidentiality will be guaranteed. This is a 24 hour a day, 7 day a week service.
