Subject Matter Expert Personal Comments: • I agree with the recommendations. • I think that their administration should be guided by a preoperative algorithm determined by their history and physical and other workup. • Their use, dosing, and length of treatment should be standardized and incorporated into a protocol. • I am not an expert in this area and can't comment on more recent evidence.
