Important Notice • e-Enrol is a tool to capture employee and dependant information for the purposes of medical insurance cover. • If the data collected through e-Enrol is found invalid, the member would not be covered. • This presentation is purely a guide on how to use the e-Enrol tool. Please refer to the policy benefits to understand their eligibility for yourself and your dependants.

How do I use e-Enrol - Step 1 - Validation • Log on to www.uhcindia.com/YourCompanyName • Supply the following details: • Company Name • Employee ID • Date of Joining • Date of Birth • Employee official e-mail id • Manager’s e-mail id (In case a employee does not have a e-mail id) • Click on Next. The data supplied by you for the items mentioned in red will be compared with the data that UnitedHealthcare India has received from your company HR. If these 2 compare positively, then your username and password will be e-mailed to you within 24 hours at the e-mail id that you have specified. Please save this email. You can use this username and password to manage your information during the policy period by logging onto www.uhcindia.com. Contd..

  5. How do I use e-Enrol - Step 2 - Add Self • Once the system validates your initial details, you would be led to the online enrolment form. • Fill the online enrolment form as required. Ensure that you enter all necessary details. • Please upload your digital photograph and click “Submit” button. If you do not have a digital photograph, click “Printable Version” after you have submitted your details. • The details filled in would be populated on the form • Please affix photographs and send to the local address corresponding to your location, as given in Slide 18. • Enrolment details for self can now be viewed . How do I use e-Enrol - Step 3 – Add dependant(s) • Use this option in case you need to add a dependant. • Select your relationship with the dependant to be added from the pull down box appearing next to “Add Dependant” tab. • Click on “Add dependant”. • You will now see the dependant information sheet. • Fill dependant details as required. • Upload a photograph of the dependant. • Click on “Submit”. • Enrolment details for self & dependents can now be viewed. • Click on “Finish” button to complete the Enrolment process. • You would receive an acknowledgment email from UnitedHealthcare India within 24 hours confirming your enrolment.

How do I use e-Enrol -– Edit self/dependant(s) • Use this option in case you need to change any details for self or dependant(s) who are already insured. • Visit www.uhcindia.com • Enter your username & password which was emailed to you during the online enrolment process to gain access to your profile. • Click “Edit” link corresponding to the individual for whom you want to make changes. • Make changes as desired and Click on “Submit”. • You have now successfully edited your details. • You would receive an acknowledgment email from UnitedHealthcare India within 24 hours confirming your changes.

How do I use e-Enroll -– Deactivate dependant(s) • Use this option in case you need to delete dependant(s) who are already insured. • Visit www.uhcindia.com • Enter your username & password which was emailed to you during the online enrolment process to gain access to your profile. • Click “Edit” link corresponding to the dependant whom you want to delete. • Scroll down and click on “Deactivate” button. • Enter valid and sufficient reason(s) for deletion and click on “Submit” button. • You have now successfully deleted your dependant. • You would receive an acknowledgment email from UnitedHealthcare India within 24 hours confirming your dependant deletion.

How do I use e-Enroll -– Upload photographs • Use this option in case you need to upload photographs of either yourself or any of your dependants. • Visit www.uhcindia.com • Enter your username & password which was emailed to you during the online enrolment process to gain access to your profile. • Click “Upload Image” link corresponding to the individual for whom you want to upload photograph. • Click “Image Formats Accepted” link to check formats accepted by the system. • Click “Browse” button to navigate to the photo image file located on your PC. • Select the photo image file to be uploaded and click “Upload” button. • You have now successfully uploaded the photograph. • Please refer to the next slide for temporary id card printing • Click “Close” button to return to your profile. • Click “Logout” button to exit your profile.

A revolutionary concept in the Health Benefit Administrative Services • A provisional identification tool • Printed from the web as a document • Only for members who e-enroll • Can be obtained with or without a photograph • Provided for each member (employee and dependant to be serviced by UHC) • An immediate solution for those who do not have digital photographs at the time of enrolment • Also for those who misplace their card Please refer to slides 11, 12, 13, 14, and 15 for screen shots.

  11. Profile Login Page Once you logon to your profile, you would be able to access your enrolment details.

  12. ID Card Status Screen Once you click the “ID card status” button, you would see the following screen:

  13. ID Card Print Screen Once you click on the Print ID Card button, you would be able to print the card even if you have not uploaded the photograph.

  14. Confirmation Screen Once you click on the print button, the system would prompt you to confirm the print job and you would not be permitted to change your name & photograph.

  15. Duplicate ID Card Request Screen Once you click on the Request for Duplicate ID Card button, an email request would be sent to our Customer Care Center. We would then dispatch a duplicate ID Card to the member. In the meanwhile you still have the option to print the digital ID Card.

  16. Online claim status • Ability to view the status of claims for the year 2005 on the web • Only for those who enrol online • All details of claims pertaining to hospitalization claims with date of admission on or after January 1, 2005 • No need to call or email Customer Service to ascertain claim status • Further details to follow
