  • Application sent to applicant

  • Application completed and, when necessary, professional verification received

  • Application mailed or faxed to LYNX

  • Reviewed for completeness and entered into database

  • TD is approved or denied based on flow chart

  • ADA & Medicaid are referred to Functional Assessment or Approved based on self certification

  • Applicant schedules Functional Assessment

  • Cognitive test performed

  • Physical test performed

  • Application transported from Functional Assessment

  • Functional Assessment emailed to LYNX

  • Eligibility determination made by LYNX staff

  • Applicant notified of approval or denial

  • Denied applicants referred to Fixed Route Travel Training

  • Application and all correspondence, documentation, functional assessment, and Medicaid verification filed by date
