  3. LTCIP Strategies Developed to Support “Vision” Communication Strategy (Aging & Disability Resource Connection ADRC) Fully Integrated Health Care Strategy-HSD+ Physician Strategy (TEAM SAN DIEGO Initiative)

TEAM SAN DIEGO • A community training created to meet the challenges of: • People living longer with diseases/conditions • An aging population • Healthcare designed for acute conditions, but… • Most healthcare dollars spent on chronic care • Pressures produced by high cost of healthcare • Individuals with chronic conditions needing more than medical care

  5. TEAM SAN DIEGO Training is a mix of on line training modules and face-to-face classroom problem-solving. The online training has eight modules that establish a common ground of understanding among participants. The classroom training puts concepts into practice with team-building, discussion of tools/techniques, and establishing “teams”.

  6. TSD Accomplishments • Engaged marketing consultant • TSD presentations in the community • Enrollment has grown steadily • In-class trainings held monthly • Waived fee for LTCIP stakeholders and all HHSA staff • Goals are aligned with national health care reform plan recommendations
