  4. Apply or re-apply to operate in the U.S. Commercial Zones using the OP-2 Application Package • Receive a provisional Certificate of Registration valid for 18 months • Receive permanent Certificate of Registration granted after 18 months

You may file online with a credit card at http://www.diy.dot.gov To obtain copies of the OP-2 Application Package: • Download a copy from the FMCSA Web site http://www.fmcsa.dot.gov/pdfs/OP-2.pdf • Request a copy from FMCSA’s Toll Free Line - U.S./Canada 1-800-832-5660 • Call a FMCSA Divisionoffice

You may file online with a credit card at http://www.diy.dot.gov • Download a free copy of OP-2 Application Package from the FMCSA Web site: http://www.fmcsa.dot.gov/pdfs/OP-2.pdf • Allow approximately 4 hours to download and print all the forms needed. • Take time to get familiar with the forms and instructions (approximately 25 minutes).

OP-2 Application Package forms and any certificates must be signed by the applicant only! • For example: • For a sole proprietorship, the owner must sign • For a partnership, one of the partners must sign • For a corporation, an official of the company must sign (President, Vice President, Secretary, Treasurer)

  8. OP-2 Application - Form OP-2 (Application Form) An original and one copy of a completed Form OP-2, with all necessary attachments and statements filled out in English and signed by the applicant. • Motor Carrier Identification Report - Form MCS-150 A completed and signed Form MCS-150, Motor Carrier Identification Report. The MCS-150 provides basic information about the carrier's operation necessary to maintain FMCSA's carrier census database. • Designate Process Agent – Form BOC-3 • List Of Process Agents - PDF Version - HTML Version • A carrier must provide a signed and dated Form BOC-3, Designation of Agents for Service of Process, or use a • process service agent who will electronically file the BOC-3 within 90 days.

  9. Even if you hired a consultant or an agent to fill out the forms, once you sign them, you are accepting liability and responsibility for the information provided!

  10. It is important that the carrier provides all required information and completes the application in its entirety. • Applications with missing information or fees will be returned to the carrier, and the process to apply starts over again. • Information provided on the application will be checked against a variety of to ensure that the carrier is eligible for a Certificate of Registration.

  11. Every motor carrier registered with the FMCSA must list the name and address of an agent of process for each U.S. border state that they may operate in.

  12. Once approved, you will receive: • A provisional Certificate of Registration that allows the carrier to operate in the U.S. Commercial Zones for a period of 18 months • A USDOT number

  13. Prior to entering the U.S.: • Mark all your commercial motor vehicles on both sides with legal name or DBA name used on Form MCS-150 and the USDOT assigned number on vehicles intended for use in the U.S.

  14. You must always have these documents in the cab of each vehicle crossing the U.S. border: • The Certificate of Registration; • The required insurance endorsement (Form MCS90); and • An insurance identification card, binder, or other document issued by an authorized insurer which specifies both the effective date and the expiration date of the temporary insurance coverage authorized by this exception.

  15. Before beginning operations: • All applicants must contact the appropriate regulatory agencies in every U.S. border state in which the carrier will operate to obtain information regarding various state rules: • Arizona - http://www.dot.state.az.us/ • California - http://www.dot.ca.gov/ • New Mexico - http://www.nmshtd.state.nm.us/ • Texas - http://www.dot.state.tx.us/

  16. Prior to the expiration of this 18-month provisional • certificate, each carrier will need to: • Pass Safety Audit • Comply with: • Federal Motor Carrier Safety Regulations http://www.fmcsa.dot.gov/rulesregs/fmcsr/fmcsrguide.htm • Federal Hazardous Material Regulations http:\\hazmat.dot.gov • All State Motor Carrier Safety and Economic Regulations http://www.fmcsa.dot.gov/links.htm#STGOV • Pass Roadside Performance Monitoring

  17. To receive a Certificate of Registration, motor • carriers will need to: • Demonstrate acceptable levels of safety performance • Receive a passing score from the Safety Audit • Receive written notification from FMCSA that the Certificate of Registration has become permanent at the end of the 18-month provisional period

  18. A carrier that has been issued a permanent • Certificate of Registration will fall under the same safety requirements and operational procedures applied to U.S. and Canadian carriers.

  20. You are responsible for making sure that the methods and processes you described in your OP-2 Application Package are kept current and accurate. • It is important to train the staff that will be responsible in administering your processes.
