  3. As your adult case manager at the Interfaith Family Shelter, I canassist you to… • Search for housing • Identify and access resources that will help you toward your goals during your stay at the shelter

  4. What to expect during our first appointment • Complete an initial adult case management intake form • Overview client responsibilities • Complete a housing assessment • Create a budget • Provide medication information for all your family members • Develop goals to be reached during your stay at the shelter

  5. After your initial intake appointment, you are required to meet with me weekly It is your responsibility to set up an appointment with me… • By phone: 425-303-9774 ext.4 • Email: casemgr@interfaith.org • In person: Monday-Friday, 9am-5pm

  6. Our weekly meetings… • Please be on time and knock loudly on my office door • If I am not in my office, please check back in 10 minutes • Your appointment is important to me and I have made this time available just for you • Meetings last about one hour

  7. In assisting you to find housing and helping you toward your goals, it is important for you to provide me with proper documents Here is a list of beginning documents I will need • Birth certificates for all family members (which I can help you to get) • Social security cards for all family members • ID cards – such as a driver’s license • Proof of income

  8. Always inform me… • In advance, if you plan to leave the shelter overnight • Changes in prescription medications • Changes in your family, income, or housing • Additions or subtractions to family members that are staying in the shelter or that are planning to join you when you secure housing

  9. Some services I can help you with or connect you to… • Workshops and classes to help you grow • Mental health concerns and referrals • Drug & alcohol concerns and referrals • Birth certificate funds • Dispute resolution and budgeting referrals

  10. Now that you know all about adult case management, let me introduce you to the other staff that are here to help you • Jackie – the shelter director that oversees all of the shelter • Constance – the shelter coordinator that assists residents by coordinating the food program, physical facilities, chores, volunteers and donations • Mr. P – the children’s advocate that acts as an advocate and supporter for the physical, educational and emotional health of your kids and youth • Oriana – the family resource advocate that mainly works with families who do not live at the shelter to lessen the need for shelter services

Remember • Set-up weekly appointments with me • Provide me with important documents • Let me know about changes in income, housing, prescription medications and additions or subtractions in family members • Attend all your appointments that have to do with housing, they are difficult to get, plan to be there I am here to help you secure housing and reach your goals during your stay at the shelter. Doing your part to keep me informed and updated will help me to assist you to move forward with your family
