  2. Neonatal Resuscitation Program Slide Presentation Kit Lesson 9: RESUSCITATION Of PRETERM NEWBORN

  3. Why do babies born preterm have a higher risk of complication? • Skin • Chest muscle • Immature lung • Immature tissue • Immature immune system • Smaller blood volume • Immature blood vessels • Limited metabolic reserve

  4. What additional resources do you need for resuscitating a preterm newborn? • Polyethylene bag(<32 w) • thermal mattresses • Servo-control radiant warmer • Oxygen blender & oximeter • ECG monitoring • Device capable of providing PEEP

  5. What additional resources do you need for resuscitating a preterm newborn? • Appropriated sized ET tube(2/5 , 3 mm) • Mask with app size • Blade (size 0 .optional 00) • Surfactant • Pre-warmed incubator

  6. How do you keep the preterm newborn warm? • Increase the room-temp 23-25 ^ C • Preheated radiant warmer • Hat • Pre-warmed transport incubator • For baby < 32 w: thermal mattresses wrap in polyethylene plastic bag

  7. How do you assist ventilation? • If breathing spontaneously • If PPV required: use lowest PIP use a device that can produce PEEP consider administering surfactant

  8. How much oxygen should be used? • 21-30 % for less than 35W • The oxygen saturation target range is same to term infant

  9. What can you do to decrease the chances of neurologic injury in preterm newborn? • Handle the baby gently • Do not position the baby legs higher than the head(Trendelenburg position) • Avoid delivering excessive pressure during PPV and CPAP • Do not rapidly infuse IV fluids

  10. What special precautions should be taken after the initial stabilization period? • Monitor the baby’s temperature • Monitor blood glucose • Monitor Apnea & bradycardia
