Our PPO network is nationwide. • Only a $25 deductible In or Out of network • Deductible is waived for Type I Preventive Services when using a network dentist • $2000 per year maximum in or out of network • Enhanced Orthodontic Lifetime Maximum increased to $2000 • Award winning Claims Center Please review the Freedom Preferred PPO Plan materials for more information. Heritage Plus with Specialty Benefit Amendment No deductibles No annual maximums for general dentistry Preexisting conditions are covered This option includes the Specialty Benefit Amendment which allows you to see in network specialists for listed services at a low co-payment. You can elect an out of network specialist and receive a reimbursement for the listed services. Orthodontic coverage is a 25% discount with a network specialist. The orthodontic benefit has no age limit and covers adults and children. Please refer to the Heritage Plus Plan with SBA materials for a co-payment listing and covered services. Heritage Secure Plan offers a very low cost plan option with rates starting at $7.20. No deductibles No annual maximums Preexisting conditions are covered Covered services have co-payments that are paid directly to the dentist. If you need specialty care on the Heritage Secure Plan you can access a Plan specialist and receive a discount. This includes the 25% discount for Orthodontic services. Adults and children are eligible for Orthodontic coverage. Please refer to the Heritage Secure Plan materials for a co-payment listing. Two Prepaid Plans Find a Prepaid network dentist at www.assurantemployeebenefits.com , under “For Members” select “Find a Dentist”, then choose “Heritage Plans”.
