The complementary products give individuals choice – the choice to select the medical coverage that will best meet their needs. • HealthSelect– coverage for day-to-day basic medical expenses • Max+ • Advantage • CriticalMed Plan – a set-level of catastrophic coverage • $25,000 • $50,000 • Dental Plan – a limited dental plan

Benefit Offering HealthSelect Value Proposition: HealthSelect offers a cost-effective way to deliver broad, affordable coverage for employees who are not covered by traditional benefit plans and are looking for help with day-to-day, basic medical expenses. • Plan Highlights: • Benefits for: doctor’s visits, emergency room, hospital stays, prescriptions, and more... • Delivers a wide range of first-dollar, specified-benefit accident and sickness coverages • Multiple voluntary coverage designs with varying benefit amounts and premium rates • DoctorNavigator: price transparency tools for provider network savings

  5. Benefit Offering CriticalMed Plan Value Proposition: CriticalMed provides a predetermined, set-limit of catastrophic coverage for an accident, hospital confinement or critical illness. It is designed for individuals who are willing to self-pay their day-to-day medical expenses. • Plan Highlights: • Benefit maximums of $25,000 or $50,000 • For members more concerned about insurance coverage for major events • Can be paired with HealthSelect as a buy-up option to round out the coverage • DoctorNavigator: price transparency tools for provider network savings

  6. Benefit Offering Dental Plan Value Proposition: The Ternian Dental Plan provides a reimbursement amount for the most common and ordinary dental procedures. The insurance coverage is designed for the minor dental work occurring most often. • Plan Highlights: • Benefit maximum of $1,000 per year • Can be paired with HealthSelect or CriticalMed as a buy-up option to round out • the coverage or as a stand-alone plan if you do not need a medical plan

  7. Enrollment Options Fill out the enrollment form on the back of the brochure Your coverage will go into effect on January 1 (even if you enroll after the 1st). TIME IS LIMITED – YOU MUST ENROLL OR DECLINE COVERAGE NOW Give your enrollment brochure to your manager or HR Department by January 15th. THIS IS MANDATORY!!!! You will need to either enroll now by completing the form, or check the declination box and sign if you do not want coverage. Management will be checking to make sure ALL employees have completed this task. NO EXCEPTIONS!
