The “Home Page” for the Site permits navigation to the pre-formatted “Risk Statements” as well as to the “Risk Statement Computer” for customized statements. A tutorial is also provided, as well as instructions for replacing outdated statements with the modified statements.

A typical Radiation Risk Statement generated by the ASP application. The “boiler-plate” template text is merged with the calculated results of comparing the EDE to a given benchmark. The completed statement is displayed in the user’s Web browser, and can be “cut and pasted” into the “Risks and Discomforts” section of a consent form document.

The on-line form is suitable for those situations where more than one procedure, perhaps involving multiple modalities, is employed in the protocol. The EDEs for individual studies are separately calculated and then added to yield a total EDE. This is compared to the benchmark and a customized radiation risk statement is created.
