Eligibility Requirements Applying organization must: • Be a nonprofit organization or have 501(c)(3) fiscal sponsor • Be located within and serve residents of LA County • Benefit underserved children age prenatal through five years • Implement project in a consortium of clinics or other consortium of sites • Impact three or more of First 5 LA’s OHN project objectives • Impact cost containment and improve quality of care and health outcomes 7

  8. Eligibility Requirements(cont’d) Applying organization must: • Choose one of the two topical areas in the RFP • Submit all required documents in the application checklist - Applicants should complete the “eligibility quiz” on the RFP website

  9. Available Funding • Applicants may request an amount not to exceed $1,000,000 per project • One award for Topic Area 1: Dental Health Information Technology Systems • Two awards for Topic Area 2: Providing Services in Non-Traditional Settings

Online Application Step 1: Register online using link from Webpage at: http://www.first5la.org/Dental-Health-Innovation-Project-RFP(also to manage account/application) Step 2: Complete eligibility questions Step 3: Upload proposal documents online • Upload project narrative and budget • Upload other documents (audited financial statements resumes, etc.) • Three forms are required to be submitted in hard copy

  11. Proposal Narrative • Summarize the project • Describe all key staff and partners • List all other funding sources and amounts for the project • Demonstrate how the project meets Level 1 and Level 2 criteria

Project Budget and Narrative • Budget amount not to exceed $1,000,000 for up to three years • Should be directly related to proposed project • Should be based on what is needed to start new project or expand existing project • Equipment should be requested in first year • 1st year contract period will be 6 months to align with fiscal year calendar
