  • Nurse drew blood from the correct patient and then postponed labeling the vial, which then resulted in a later incorrect labeling.
  • Steps:
    • Someone from the parents floor must collect the blood and verify the accuracy of the blood type and the patient.
    • Then two clinicians have to verify that the correct blood type is being administered.
  • JCAHO suggests some help to reduce risk of error
