Chikungunya Infection ในประเทศไทย • พ.ศ. 2501 ระบาดครั้งแรก ตรวจพบที่กรุงเทพมหานคร(40,000/2,000,000) 2% • พ.ศ. 2519 จังหวัดปราจีนบุรี • พ.ศ. 2531 จังหวัดสุรินทร์ จังหวัดสงขลา • พ.ศ. 2534 จังหวัดขอนแก่น(96) • พ.ศ. 2536 จังหวัดเลย และจังหวัดพะเยา • พ.ศ. 2538 จังหวัดนครศรีธรรมราช (576) และจังหวัด หนองคาย(331)

  4. 82 คน,,Median age =54 ปี (range=8-81), < 15 yr old= 2

CHIK Fever IP : 2-3 days (1-12 days) Clinical Feature: - Fever, acute onset, high 2-4 days - Joint pain / swelling - Skin rash(MP) +/- itchy - headache / retro-orbital pain - myalgia - GI symptoms/signs - Petechiae (+/- tourniquet test) Laboratory:CBC- normal - leukopenia, norm.platelet Treatment: Symptomatics.

  8. อาการและอาการแสดง ไข้สูงเฉียบพลัน ปวดข้อ ผื่นแดง และอาการทั่วไปอื่น ๆ เช่น ปวดศีรษะ ปวดเมื่อยตามตัว อาการจะคงอยู่โดยเฉลี่ยประมาณ 7 วัน ระยะฟักตัวของโรค (incubation period) 2-3 วัน( range of 1-12 days.)

  9. ข้อมูลผู้ป่วยในจังหวัดนราธิวาสข้อมูลผู้ป่วยในจังหวัดนราธิวาส • อุณหภูมิเฉลี่ย 39.1 c ไข้สูงมากกว่า 39 c ถึง 50% ไข้มากกว่า 40 c พบถึง 17% • พบอาการชักจากไข้สูงในเด็กได้บ่อยกว่าไข้เลือดออก (Chik 15% VS Den 5 %)

  10. ลักษณะการปวดข้อ • polyarthralgia มากกว่า arthritisมักพบในข้อเล็กๆ เช่น ข้อมือ เท้า ข้อนิ้วมือ เท้า แล้วถึงไปที่ข้อใหญ่ เช่น ข้อเข่า ข้อศอก ข้อไหล่ ข้อสะโพก.

  19. Treatment • Chikungunya fever is not a life threatening infection. • Symptomatic treatment for mitigating pain and fever using anti-inflammatory drugs along with rest usually suffices.

  20. การดูแลรักษา ไม่มีการรักษาเฉพาะ เป็นการรักษาตามอาการ โรคจะหายเองได้ การรักษาตามอาการ Supportive care การพักพร้อมการทำกายภาพเบา ๆ เพื่อป้องกันข้อยึดติด

  21. สิ่งที่ควรคำนึงในการรักษา(1)สิ่งที่ควรคำนึงในการรักษา(1) • การใช้ยา non-steroidal anti-inflammatory drugs (NSAID) ในกรณีปวดข้อ : ระวังในผู้ป่วยโรคกระเพาะ โรคไต และโรคหัวใจ • ไม่แนะนำการใช้ steroids ในระยะเฉียบพลัน เพราะมีผลข้างเคียงมากกว่า

  22. การดูแลรักษา(ต่อ) • รายที่มีอาการปวดข้อเรื้อรัง (ร้อยละ 12) คือปวดข้อต่อเนื่องกันนานมากกว่า 6 สัปดาห์ • การรักษาแนะนำการให้ทำกายภาพบำบัดเพื่อป้องกันข้อติดและการใช้ NSAID เพื่อลดอาการปวดข้อ

  23. สิ่งที่ควรคำนึงในการรักษา (2) • กรณีปวดข้อ อาการไม่ดีขึ้น แนะนำส่งต่อให้ แพทย์โรคข้อและรูมาติสซั่ม ดูแล • การใช้ chloroquin วันละ 250 mg ร่วมกับ NSAID ต้องระวังผลข้างเคียงระยะยาวของ chloroquin ต่อดวงตาและการมองเห็นได้

  24. คำแนะนำสำหรับโรงพยาบาลคำแนะนำสำหรับโรงพยาบาล • กรณีที่ผู้ป่วยต้องนอนโรงพยาบาลต้องระวังยุงกัดผู้ป่วยและจะแพร่โรคไปยังผู้ป่วยรายอื่น รวมถึงบุคลากรอาจจะติดโรคได้ อาจจะต้องใช้มุ้ง หรือมุ้งลวดตลอดระยะที่มีไข้ • แนะนำให้โรงพยาบาลทุกแห่งต้องทำลายแหล่งเพาะพันธ์ยุง และแจกยาทากันยุงให้ผู้ป่วย 5 วัน นับตั้งแต่วันเริ่มป่วย
