  1. De Anza College Nursing Program Almost everything you need to know but forgot after the info session!!

What to Bring the First Day? • Your completed health form: -yes, it must be signed by a health professional…(your signature won’t do!) • Documented immunizations within the allowable dates • Evidence of a CPR course (BCLS-C: for health professionals) • Proof of student malpractice insurance (on your fees statement)

  3. Women: -blue top worn with a white shirt (must have a collar, with short or ¾ length sleeves) -white pants (not denim) or skirt -all white shoes -white socks/ beige or white stockings Men: -blue top worn with a white T-shirt -white pants -all white shoes -white socks What about my uniform???

  4. I forgot….who do I call about the uniform? Don’t Panic!!!!! Dove Uniforms will come to the first class. FYI: 1-800-345-5555
