  1. HED and Admin-RxModify/Inactivate Report • Shows all HED/Admin-Rx entries that are altered or inactivated after having once been entered and saved (if someone entered something in error, noticed it before they saved, and either changed or removed, this would NOT be on this report but on the Near Miss Report in Business Objects) • Practical application: • Narcotics diversion – when documentation may be altered to cover up diversion • For Root Cause Analysis when timing and sequence of events may be critical • For documentation audits for other causes

  2. O = original entry; if perform date/time & chart date/time differ, it will display both. Includes last 4 characters of RacF ID as “Chart Inits.” M = Modification (date/time something altered & what alteration was) I – Inactivation (date/time something removed)

  3. Patients discharged more than 48 hrs. ago, go to StarPanel Modify/Inactivate report is in All Docs view on date of admission with other HED reports. Patients still in house or discharged less than 48 hrs. ago, go to HED click on Report, then Administrative Reports
