  3. Logging In • You should be in the system (having prepared one prior to this deployment). • Your Login ID is your full SSN (no dashes) • If you can’t remember your password, no worries, Use the “Forgot Your Password?” link Click Here

  4. If you forgot your password…… • You will be asked a series of questions used to identify yourself • One item that some people get tripped up on on is rank • The system only knows the rank you held the last time you used the system • So, if the website is having trouble finding you, try with one or two previous ranks • Last resort …… • Talk to Medical

  5. Once you are logged in…. . . . . . . . You will see all your past surveys. • **IMPORTANT: • Find the PRE-deployment health assessment you completed before this deployment (GREEN ARROW). • IF there is more than one from 2010 – choose the one that says “Yes” in the “Certified” column (YELLOW ARROW). • To the right, you will see the associated POST-deployment health assessment. Click on the “edit” button. (RED ARROW).

  6. Using the PDHA Form • There are 5 pages – please make sure you complete each page • To help you navigate the system, see below Moves back to the FIRST page Moves you on to the Next page Moves back one page These will move you to that Individual page

Completing the PDHASome IMPORTANT items to remember……… PAGE 1:Demographic Info • UIC= 67105 (2d MSB Fwd) • This is important or we will have a hard time finding your form to review! • Deployment country = Afghanistan • Name of operation = OEF • Current Contact info = Home/Squadron address in CONUS

  9. Completing the PDHASome IMPORTANT items to remember……… PAGE 5: • Question 22: • List any immunizations you received (e.g. Anthrax, Smallpox – it’s ok if you don’t remember them all) • Question 23: • Check the “Doxycycline” box • Answer the YES or NO question • Answer Questions 24-27

  10. When Completed 3rd 1st 1st Save the form ! 2nd Once it is saved you may see a validation on the top of the screen. It will let you know if you left any required questions blank. 3rdIf you are good to go, you can close the form. *** Do NOTprint the form. The Medical Officer reviews all assessments and will print it afterwards.

  11. DONE? Thank you! When you are done, have made any changes you need to, click to Logoutand you are done. Please let your shop OIC know when you are done. Each Marine will sign up for a time to see the Medical Officer to complete the assessment.
