  2. The Whole Senior Connection™ is a web-based program that helps you discover what services may benefit you. The Whole Senior Connection™ enables you to contact the providers of those services that you decide are best for you, conveniently and anonymously. There are only four steps to follow: Completing a Profile, Reviewing & Selecting Services & Providers, Creating a Success Plan, Getting Connected. Introduction

  3. Finding services is easy… Go to a computer and access the Internet. Getting Started

  4. Getting Started Log into the Whole Senior Connection™ as a new or returning user.

  5. Completing A Profile Complete the Senior Profile, or Survey. Answer each of the questions by selecting the appropriate “Yes” or “No” response. These questions will help identify the services that you may need.

  6. Reviewing & Selecting Services Select one or more services to see the appropriate providers. If you would like to see a list of providers for one or more services, select the checkboxes next to those services. You must select at least one service to proceed. When you have finished selecting services, select “Display Provider Details” at the bottom of the page to view the information and organizations that provide the services you selected. You may return to this screen and change your selection later.

  7. Creating Your Success Plan Create your personal Success Plan. Your Success Plan is a summary listing of all service providers you selected. The purpose of the Success Plan is to provide you with a complete list of service providers to address all of your selected needs. After you have reviewed the information listed, select the boxes of each provider you would like to add to your Success Plan.

  8. Creating Your Success Plan You may either automatically notify the provider that you are interested in meeting with them, or if you prefer, simply add them to your Success Plan and contact them later, if more convenient.

  9. Creating a Success Plan When you have finished your provider selections, press "Submit" at the bottom of the page. You may also return to the Analysis page and select different providers by using "Back" on your browser. Your Success Plan has now been created.

  10. Getting Connected Please remember that entering your contact information while completing your Profile is optional. If you choose to stay anonymous, you must contact the service providers directly at your convenience. Or, if you prefer, simply enter an email address and/or phone number so the provider may contact, or connect with, you. You have now made the connections to the services you needed. Note: A service provider will not contact you unless you have given them permission and the contact information to do so.

  11. Deciding Next Steps Decide your next steps. You may complete additional Profiles, for other family members, or take part in our User Satisfaction Survey.

  12. Summary With the Whole Senior Connection™ you choose your level of participation and, therefore, the level of services. You can remain anonymous. Service providers have been pre-qualified, and many are certified as “Elder Friendly” by We Help Brevard, Inc. The Whole Senior Connection™ is available at no cost 24 hours a day, everyday, including weekends and holidays.
